Our Clinic helps family physicians manage the growing burden of chronic pain through structured assessment, evidence-informed treatment plans, and improved access to allied health services.

Recommend alternative treatment pathways for opioid dependency

Support AISH applications by providing collateral medical information

Access structured chronic pain assessments and individualized treatment plans

Improve continuity of care for patients living with chronic pain

Assess stability of comorbidities, including mood disorders and insomnia

Assist with medication optimization and appropriate referral pathways

Offer complimentary physiotherapy treatment for your patients
What do we NOT do?

We do not Start, Change, Titrate, or Refill existing opioid regimens.

We do not Taper opioid regimens.

We do not assume Primary Care responsibilities for the patients in our program.

We do not treat chronic abdominal pains, unless the patient has also seen Gastroenterology.

We do not currently treat chronic migraines.

We do not fill AISH applications.

We do not refill medications that we do not prescribe.
How to we refer?

We accept referrals from Physicians, Nurse Practitioners, and Physiotherapists.

Write us a referral letter using your EMR's standard template to include as much Past Medical History, Medications, Allergies, Social History, and Surgical History as possible.

Tell us about your treatment goals for the patient, if any.

Tell us about any non-controlled medications such as psychiatric medications that you prefer that we do not modify.
